They are designed to work together — the annex references each attachment by letter, and each document can be updated independently as your district’s plans evolve. Pre-ops, during operations, and close-of-operations checklist for the Reunification Director. Three phases, every critical decision point covered.
Site Setup Checklist Pack
Four area-specific setup checklists — Greeting, Check-In, Reunification, and Student Assembly. One per area supervisor, completed before authorized adults arrive.
Job Action Sheet Pack
Nine role-specific job action sheets covering all supervisors and front-line staff. Each includes Pre/During/Close phases, process flow, and Do/Don’t guidance.
Student Accountability Roster
25-student roster with present/absent columns, notes, and signature block. Completed immediately upon evacuation and delivered to the Reunification Director.
Student Release Log
20-entry release log capturing student name, authorized adult name, ID type and number, time, and staff initials. One per Check-In station. A legal record — do not destroy.
Notification Script Pack
Six scripts covering every communication phase — initial activation, site open, delay update, all clear, staff notification, and extended hold. Each in SMS, email, and PA formats.
Authorized Adult Guide to Reunification
Plain-language guide explaining what reunification is, what to bring, what to expect at each step, and what not to do. Distribute before an incident and hand out at the site.

Categorized inventory with quantity-needed and quantity-on-hand columns across six supply categories. Sized for a 500-student school with guidance to adjust for your enrollment.
Site MOU Template
Complete memorandum of understanding template for off-site reunification locations. Site description table, terms and conditions, and execution signature blocks. Review with legal counsel before use.
After Action Report
Seven-section AAR template covering strengths, areas for improvement, observations by area, improvement plan with assigned actions and due dates, and quantitative summary. Complete within 48–72 hours.
Annex Self-Evaluation Checklist
Six-category evaluation checklist covering annex currency, team readiness, site agreements, operational materials, training, and partner coordination. Complete annually and after every exercise.
